Nominations for these awards require submission by February 1st, 2012 for review and consideration by the Council on Nominations, Awards & Education. Nomination forms are available on the MDA website July 1, 2011 - February 1, 2012. Selected winners will be announced and awards will be presented during the 2012 MDA annual session.
Required: The person making nomination must be MDA members, associate members, employees of member dentists, or members of Dental Related Professions. Exp: Dental Lab, Mission First Clinic, DDS Coordinator or UMC SOD / Alumni employees. Must include their name and contact information when completing nomination form.
Note that supporting documents must be included with each nomination, example: newspaper clippings, articles, photos etc. and should be mailed to the MDA office.
