A Continuing Education registration form should be completed to register “stand alone” or “one time” CE courses with the MDA. Each CE course must be registered at least two weeks prior to presentation to qualify for certification through the MDA. Completed forms should be returned along with a check for the registration fee to:

The cost to register a CE course is:
$50 course length = 30 minutes - 5.5 hours
$100 course length = 6 hours - 10.5 hours
$150 course length = 11 hours or more
Be sure to include information that would clarify credentials along with references when registering.
Applications for registration will be forwarded to Dr. Mindy Austin, Chairman of the MDA Council on Nominations, Awards & Education for approval. A signed copy of the registration form along with roster to be signed by all attendees will be returned to you once the application has been approved and processed. Please note that it is the program provider's responsibility to provide proof of attendance or CE verification cards to all program attendees. CE verification forms that you may copy and use for this purpose will also be provided by the MDA.
