MDA Login Instructions

Here are the steps that you will need to follow:   

Refer to your ADA membership card, this is the guide for how your name has been entered into the database system. (FYI..this cannot be changed.)

First blank box -  user name = enter your complete ADA member number....no spaces.

• Example: 123456789

Second blank box – password- enter your last name only - Capitalize the first letter, no spaces.
(If your last name includes a ‘ be sure to include that as well)

• Example:   John M Dentist  =  Dentist
  
  
• Example:   John M O’Dentist = O’Dentist
  
  
• Example:   John M McDentist, Jr. =  McDentist
